Sexual anxiety and performance fear in Palmones, near Algeciras, Sotogrande and Gibraltar

Sexual anxiety can appear when intimacy stops feeling calm and starts to feel like a test. There may be desire, affection and attraction, but also fear of failing, excessive control, bodily block or avoidance.

At Ocnos Psychology Clinic, we work with sexual anxiety from a psychological, confidential and non-judgemental perspective, helping you rebuild a sense of safety in intimacy without turning sexuality into a performance obligation.

The main mechanism

The cycle of sexual anxiety

Sexual anxiety is often maintained by a cycle that repeats itself. The more you try to control the sexual response, the more attention goes to the body; and the more attention goes to the body, the harder it becomes to feel calm, desire, arousal or connection.

This cycle can affect men and women, whether in a relationship or in occasional encounters. It can also overlap with premature ejaculation, erection difficulties, low sexual desire, bodily blocks, pain, anorgasmia or avoidance of intimacy.

Sexual anxiety often begins before intimacy, as anticipation and fear that it might happen again.
1

Anticipation

The thought appears: “What if it happens again?”, even before the intimate situation starts.

2

Self-monitoring

Attention shifts to the body: erection, arousal, lubrication, ejaculation, orgasm or pain.

3

Pressure

Sexuality stops feeling like connection and starts to feel like an evaluation.

4

Block

The body may respond less freely precisely because it is being controlled too much.

5

Avoidance

To avoid feeling shame or frustration again, the person may start avoiding intimacy.

6

More fear

Afterwards, the experience may be remembered as a failure, increasing fear for next time.

Before, during and after

Sexual anxiety does not only happen during sex

An important difference in this page is that we are not focusing on one single symptom, but on the full anxiety process that may appear before, during and after intimacy.

Before

Anticipation and fear

There may be unease, avoidance, repetitive thoughts, comparison with past experiences or fear of disappointing the other person.

During

Control and self-monitoring

Attention becomes so focused on “doing it right” that the person disconnects from pleasure, the body and the relationship.

After

Guilt and mental replaying

After the encounter, excessive analysis, shame, frustration or the feeling that next time will be the same may appear.

Understanding the difference

Not every sexual block means lack of desire

A person may desire their partner and still feel blocked. They may want intimacy and, at the same time, avoid it because they fear their body will not respond as expected.

That is why it is important not to interpret every block as lack of love, lack of attraction or disinterest. Sometimes desire is present, but it is covered by anxiety, bodily monitoring, past experiences or fear of disappointing the other person.

In therapy we do not look for blame. We try to understand whether the main issue is sexual anxiety, low sexual desire, relationship tension, a specific sexual difficulty or a combination of several factors.

Body and control

When the body stops responding because of too much control

The sexual response does not work well under threat. When the mind experiences intimacy as a situation of evaluation, the body can go into alert mode. This may lead to less arousal, erection difficulty, premature or delayed ejaculation, pain, dryness, difficulty reaching orgasm or disconnection.

That is why psychological therapy is not about demanding more control from you. It is about helping you reduce the alarm, return attention to the present and build a kinder relationship with your body.

Our approach

How we work on safety, not performance

Sexual anxiety often gets worse when the person tries to “prove” something. That is why, at Ocnos Psychology Clinic, we work so that intimacy stops feeling like an exam and can become a space of safety, communication and connection again.

1

Understanding the pattern

We explore when anxiety appears, what activates it, how the body responds and which attempted solutions may be maintaining it.

2

Reducing self-monitoring

We work with the tendency to monitor the body, anticipate failure or constantly measure the sexual response.

3

Rebuilding safety

We practise emotional regulation, attention to the present, intimate communication and gradual exposure when appropriate.

4

Integrating changes

We review progress, obstacles and personal or relationship needs so that the process remains realistic and useful.

Therapy format

Individual therapy or couples therapy

Sexual anxiety can be worked on individually when the person needs to understand their block, reduce shame, improve their relationship with their body or rebuild safety before speaking about it with their partner.

It can also be worked on as a couple when anxiety has become distance, silence, pressure or avoidance. In that case, the aim is not to find fault, but to create a safer way of talking about intimacy, desire, rhythms and needs.

If you are not sure which format is right for you, we can guide you during the first session.

Individual session

€70 / 60 min

To work on anticipatory anxiety, self-monitoring, shame, avoidance and bodily safety.

Couples session

€110 / 90 min

To address intimate communication, pressure, fear of failure, desire and disconnection in the relationship.

The aim is to recover a calmer, safer and more connected experience of intimacy.
What to expect

What we usually work on in the first sessions

In the first sessions, we try to understand the full map of sexual anxiety. You do not need to have it all clear or be able to explain it perfectly: our work is to help you organise what is happening carefully and without judgement.

  • History of the problem: when it started, where it appears and what changes it has produced.
  • Anticipatory thoughts: fear of failure, comparison, pressure or mental replaying.
  • Bodily response: tension, block, disconnection, difficulty with arousal, erection, lubrication or orgasm.
  • Avoidance: behaviours that relieve anxiety in the short term but maintain fear over time.
  • Relationship context: communication, expectations, silences, excessive care-taking or frustration.

Hector is amazing. As a parent, supporting a 10-year-old child through life’s challenges sometimes feel overwhelming, and having the right guidance makes a big difference. He is helping me better understand my daughter needs, and how I can best support her as a parent. What I appreciate most is that he doesn’t just offer advice; he helps me see situations from a different perspective. His guidance is really valuable to me and has made a big difference in the way I approach parenting. I highly recommend him. Thank you Hector.

Booking my first appointment with Hector was one of the best choices I made. It took me many years to take the first step, and I am very glad I did, and that it was Hector the person helping me on my journey to overcome the obstacles preventing me from moving forward in my life. With just a few sessions and proper guidance and self work I made a lot of progress that I am satisfied with. I wanted to help myself, but I needed a guide to help me help myself. Hector was that person. Very grateful and thankful, and highly recommended. Thank you 🙏

Ocnos Psychology in Palmones is a truly welcoming, professional clinic, with a calm environment and ample parking that makes visiting feel easy from the outset. I have personally found Hector’s support invaluable in deepening my understanding of neurodiversity and developing strategies that have helped me both personally and professionally. He is kind, compassionate, highly knowledgeable and wonderfully adaptive to each person’s needs. He creates a safe space where you feel genuinely heard, supported and empowered to build on your strengths. He offers sessions in both English and Spanish, and his background as a teacher makes him exceptionally well placed to work with both children and adults. The highest compliment I can give is that, as a coach myself, I would actively refer clients to Hector if they wished to explore a neurodiversity assessment or diagnosis. Highly recommended.

I had the best experience at Ocnos Clinic. I had my sessions in English with Héctor, and from the very first session I felt comfortable, listened to, and genuinely understood. What stood out to me most was how knowledgeable and dedicated Héctor is. It’s clear that he genuinely cares about his clients and goes above and beyond to help. His approach has really helped me better understand and manage my anxiety, and I’ve already noticed a positive difference after just a few sessions. I would highly recommend Héctor and Ocnos Clinic to anyone looking for professional, compassionate support. It’s absolutely worth it.

Frequently asked questions

Frequently asked questions about sexual anxiety

Understanding sexual anxiety

What is sexual anxiety?

Sexual anxiety is a response of fear, pressure or block that appears before, during or after intimacy. It can lead the person to monitor themselves too much, anticipate failure, avoid sex or disconnect from pleasure and from their body.

Why do I get nervous before sex if I do have desire?

Because desire and safety are not the same. You may have desire, feel attraction and want intimacy, while also fearing failure, disappointing the other person, blocking or repeating a previous experience. In these cases, anxiety can cover the desire.

Does sexual anxiety mean I do not want my partner?

Not necessarily. Sexual anxiety can appear even in relationships with affection, desire and commitment. Sometimes the problem is not the relationship itself, but pressure, self-monitoring, past experiences or fear of not responding as expected.

Symptoms and bodily block

Can sexual anxiety affect erection, ejaculation or orgasm?

Yes. Anxiety can interfere with the sexual response and contribute to erection difficulties, premature or delayed ejaculation, reduced arousal, difficulty reaching orgasm, pain, dryness or bodily disconnection. That is why it is important to assess the whole mechanism, not only the visible symptom.

What happens if I monitor myself too much during sex?

When a person monitors themselves too much, they may stop being present in the experience. Attention focuses on checking whether the body responds, whether they are doing it right or whether the other person is satisfied. This monitoring usually increases anxiety and makes the sexual response more difficult.

Is it better to avoid sex until I feel ready?

Avoidance may relieve anxiety in the short term, but in the long term it often maintains fear. In therapy, no one is forced into sudden exposure; we work gradually to rebuild safety, communication and a less threatening relationship with intimacy.

Treatment and sessions

Is sexual anxiety better worked on individually or as a couple?

It depends on the case. Individual therapy can be useful for working on shame, fear, self-monitoring and bodily safety. Couples therapy may be recommended when there is pressure, silence, distance, conflict or difficulty talking about intimacy.

Does therapy include sexual exercises in the consultation room?

No. No sexual practices take place in the consultation room. The work is based on clinical conversation, psychological assessment, psychoeducation, emotional regulation, communication and adapted guidelines to practise outside sessions when appropriate.

How much does a session cost?

The individual session lasts approximately 60 minutes and costs €70. If working as a couple, the session lasts approximately 90 minutes and costs €110.
