Psychological treatment for erectile dysfunction in Palmones, near Algeciras, Sotogrande and Gibraltar
Erectile dysfunction can be experienced with worry, embarrassment or fear of failure. However, it is not always explained only by the body. Sometimes anxiety, pressure to perform, self-confidence, stress or the way a person experiences intimacy can play an important role.
At Ocnos Psychology Clinic, we offer psychological support for erectile dysfunction in a confidential, professional and non-judgemental space. We are based in Palmones, with easy access from Los Barrios, Algeciras, La Línea, San Roque, Sotogrande and Gibraltar.
When the body responds to fear, pressure or self-demand
Psychological erectile dysfunction often appears when a person wants to have sex, but their body enters a state of vigilance, tension or blockage. As a result, the more they try to control the response, the more pressure can appear.
Sometimes it follows a specific experience that brought embarrassment or distress. In other cases, it is linked to stress, relationship problems, general anxiety, depression, low self-esteem, tiredness or fear of not being “good enough”. It can also coexist with medical factors, so the situation needs to be assessed carefully.
Key idea: having an erection difficulty does not mean lack of desire, lack of masculinity or lack of attraction. It is a signal that deserves to be understood, not judged.
When to ask for help with psychological erectile dysfunction
An isolated episode can happen because of tiredness, alcohol, stress or temporary pressure. However, it is worth seeking help when the difficulty repeats itself, creates anticipatory fear or starts to affect sexual and emotional wellbeing.
Fear before intimacy
When anticipation, body tension or thoughts such as “it will happen again” start to appear.
Avoidance of intimacy
When you begin to avoid sexual situations in order not to face embarrassment, pressure or distress.
Distance in the relationship
When the difficulty becomes silence, insecurity, blame or a loss of spontaneity between partners.
Important: erectile dysfunction can have psychological, physical or mixed causes. If it appears suddenly, if there is pain, urinary symptoms, diabetes, high blood pressure, cardiovascular problems, hormonal changes, new medication or substance use, we recommend also speaking to a GP or urologist. Psychological therapy does not replace medical assessment when this is needed.
How it can affect self-confidence and the relationship
Erectile dysfunction does not only affect the sexual moment itself. It can also affect self-confidence, body safety, desire, communication and the way someone approaches their partner.
Some people begin to monitor themselves too closely, as if they had to constantly check whether everything is “working”. Others expect failure before anything has even started and become blocked in advance. When the subject is not spoken about, a partner may interpret silence as lack of desire, rejection or emotional distance.
That is why therapy works with both individual anxiety and the relationship context. The aim is not to perform. The aim is to recover calm, confidence and safer intimacy.
How we work at Ocnos Psychology Clinic
We do not use the same formula for everyone. First we assess what is happening. Then we build a realistic, respectful and tailored therapy plan.
Initial assessment
We explore when the difficulty appears, how long it has been happening, in which situations it improves or worsens, and which medical, emotional or relationship factors need to be considered.
Anxiety and anticipation
We work on fear of failure, body monitoring, pressure to perform and the thoughts that increase tension before or during intimacy.
Confidence and body safety
We introduce tools to reduce self-monitoring, strengthen body confidence and recover a less demanding experience of sexuality.
Intimate communication
When there is a partner, we help you talk about the difficulty without blame, without pressure and in a way that does not increase the blockage.
Individual therapy or sex therapy for couples
Individual therapy may be appropriate when anxiety, shame, self-confidence, previous experiences or fear of failure are the main factors. Sex therapy for couples can be useful when the difficulty is already affecting desire, communication or the relationship.
In many cases, starting individually and considering a joint session later can be a good option. However, if the relationship is already significantly affected, it may be helpful to address the issue together from the beginning.
Individual session
€70 / 60 minFor sexual anxiety, fear of failure, self-confidence, shame or avoidance.
Couples session
€110 / 90 minFor intimate communication, pressure, desire, frustration or the impact on the relationship.
What we usually work on in the first sessions
The first sessions are not about forcing quick solutions. They are about understanding the pattern, reducing pressure and starting to rebuild confidence gradually.
- Understanding whether the difficulty appears always or only in certain contexts.
- Exploring sexual anxiety, stress, fear of failure or avoidance.
- Looking at self-confidence, shame, sexual history and previous experiences.
- Checking whether medication, physical symptoms or medical factors may be relevant.
- Working on intimate communication if the relationship has been affected.
- Defining clear and realistic goals for your therapeutic process.

Asking for help with a sexual difficulty can bring up many doubts. Reading other patients’ experiences can help you understand how we work: with warmth, professionalism, confidentiality and without judgement.
When erection difficulties appear alongside anxiety or relationship pressure
In some cases, erection difficulties appear together with sexual anxiety, fear of performance, premature ejaculation or tension in the relationship. It can therefore be useful to assess these aspects together.
Frequently asked questions about psychological erectile dysfunction
Causes and initial assessment
Can erectile dysfunction be psychological?
Yes. Erection difficulties can be linked to sexual anxiety, fear of failure, stress, low self-confidence, pressure to perform, relationship difficulties or previous sexual experiences that were felt as distressing. However, erectile dysfunction can also have physical or mixed causes, so each case should be assessed carefully.
When should I also speak to a GP or urologist?
It is advisable to speak to a GP or urologist if the difficulty appears suddenly, if there is pain, urinary symptoms, cardiovascular problems, diabetes, hormonal changes, new medication, substance use, or erection difficulties that occur frequently in different situations. Psychological therapy does not replace medical assessment when this is needed.
Treatment and session format
Can erectile dysfunction be worked on without medication?
In many cases, when psychological factors are significant, therapy can help reduce anxiety, rebuild confidence, work on anticipatory thoughts and recover a calmer experience of intimacy. In other cases, medication may be useful and should be assessed by a medical doctor.
Is it better to come alone or with my partner?
It depends on the case. Individual therapy may be appropriate when anxiety, shame, self-confidence or fear of failure are the main factors. Couples therapy may be useful when the difficulty is affecting communication, desire, intimacy or the relationship. It is also possible to start individually and consider a joint session later on.
Price, duration and expectations
How much does a session cost?
An individual session lasts approximately 60 minutes and costs €70. If the work is carried out as a couples session, the session lasts approximately 90 minutes and costs €110.
How many sessions are usually needed?
There is no fixed number. It depends on whether the difficulty is recent or long-standing, whether it appears only in some situations, whether anxiety is intense, whether there is a partner involved, and whether there are medical or relationship factors. After the first assessment, we can give you clearer guidance about the treatment plan.
Will therapy feel uncomfortable?
It is normal to feel embarrassed at first. Therapy takes place with respect, confidentiality and without judgement. No sexual practices take place in the consultation room. The work is based on clinical conversation, psychological assessment, guidance and tailored exercises to do outside the session when appropriate.
Can therapy improve the relationship?
Yes. When erectile dysfunction is creating avoidance, pressure or emotional distance, therapy can help improve communication, reduce blame, talk about desires and limits, and recover intimacy that is less focused on fear of failure.
