Understanding before intervening

What delayed ejaculation is

Delayed ejaculation refers to persistent difficulty ejaculating, or needing a long time, effort or stimulation to do so. It is not defined only by minutes. What matters is whether it causes distress, frustration, avoidance, pain, tension in the relationship or a sense of disconnection.

Some people can ejaculate when they are alone but not with a partner. Others can only do so in one very specific way. It can also appear after a period of stress, a medication change, low mood, difficult sexual experiences or changes within the relationship.

Key idea: delayed ejaculation is not only about taking longer. It is about understanding what may be blocking the sexual response: pressure, anxiety, habits, body disconnection, medical factors or a combination of several elements.

When the difficulty repeats itself, the pressure to reach climax can end up blocking the sexual response even more.
The pattern that often keeps the problem going

The delayed ejaculation cycle

In therapy, we often see that people do not struggle to ejaculate because they “do not want to” or because they “are not attracted to their partner”, but because they enter a cycle of effort, monitoring and pressure that makes the body’s natural response more difficult.

1

Expectation

The thought “let’s see if I can finish today” appears even before intimacy begins.

2

Self-monitoring

Attention moves towards performance, time, the partner’s reaction or bodily sensations.

3

Effort

The person tries to control the sexual response, but that control can increase tension.

4

Frustration

If ejaculation does not happen, tiredness, guilt, comparison or fear of disappointing the other person may appear.

5

More pressure

The next encounter starts with more vigilance, and the cycle can repeat itself.

It does not always happen in the same way

When the difficulty appears only in some situations

One of the key parts of the assessment is understanding where the difficulty appears and where it does not. This helps us differentiate whether the block is more related to anxiety, type of stimulation, relationship context, medication, mood, sexual habits or physical factors.

With a partner, but not alone

This may suggest that pressure, self-monitoring, fear of taking too long or intimate communication are playing a role.

Only with one specific type of stimulation

Sometimes the body has become used to a very specific pattern, which can be difficult to transfer to shared intimacy.

After medication or life changes

Some medicines, periods of stress or health problems can change the sexual response.

With frustration in the relationship

The difficulty can become a sensitive issue if it is experienced as rejection, obligation or failure.

Responsible assessment

Factors worth exploring

Delayed ejaculation can have psychological, relational, medical, pharmacological or mixed causes. That is why we do not work with quick explanations. First, we understand the context; then we suggest a realistic plan.

Anxiety and control

Self-monitoring, pressure to finish, fear of disappointing, comparison or difficulty letting go.

Relationship and communication

Silence, guilt, expectations, tension in the relationship or fear that the other person may interpret it as rejection.

Habits and stimulation

Very specific masturbation patterns, pornography use, rigid routines or difficulty adapting the body to another situation.

Health and medication

Antidepressants, alcohol, neurological conditions, diabetes, urological surgery, age or other factors that may need medical review.

Important: if the difficulty appears suddenly, coincides with a medication change, or there is pain, urinary symptoms, changes in sensitivity, previous medical conditions or any medical concern, we recommend speaking with your GP or a urologist. Psychological therapy does not replace medical assessment when this is needed.

How we work

Psychological support for delayed ejaculation

Treatment is not about forcing ejaculation or turning intimacy into a task. Our aim is to help reduce pressure, understand the pattern, rebuild body connection and improve communication when there is a partner.

Each process is adapted to the person’s sexual history, current life stage, relationship context, general health and therapeutic goals.

Talking about this with a professional can reduce pressure and help organise what is happening.
1

We assess when it happens and when it does not

We explore whether it happens with a partner, alone, in every sexual encounter, from the beginning or after a specific period.

2

We identify pressure, self-monitoring and avoidance

We work on anticipatory thoughts, excessive effort, fear of disappointing and strategies that may be keeping the block going.

3

We review habits and body context

When needed, we explore stimulation routines, body disconnection, stress, tiredness or patterns that are difficult to transfer to shared intimacy.

4

We introduce gradual tools

Strategies are introduced progressively, without forced exposure and always adjusted to the person or couple.

5

We measure progress without turning it into an exam

We review changes in anxiety, communication, enjoyment, pressure and sense of control so the process remains useful and realistic.

When there is a partner, talking without blame or criticism can reduce pressure and open a safer form of intimacy.
Individual or couples therapy

Should I come alone or with my partner?

Delayed ejaculation can be worked on individually when the person wants to understand their pattern, reduce pressure, review habits, work on anxiety or rebuild body confidence.

Couples therapy can be useful if the difficulty is already creating distance, doubts, frustration or painful interpretations. In that case, the aim is not to look for blame, but to create a space to talk about desire, timing, expectations and boundaries with more care.

What to expect

What we do and do not do in therapy

We know that asking for help with a sexual difficulty can feel embarrassing. That is why we explain the process clearly from the beginning.

What we do

  • Clinical and confidential listening.
  • Respectful psychological and sexual assessment.
  • Psychoeducation about sexual response, pressure and habits.
  • Home-based strategies when appropriate.
  • Communication work if there is a partner.

What we do not do

  • There are no sexual practices in consultation.
  • No exposure is forced.
  • We do not judge habits, sexual history or orientation.
  • We do not replace a necessary medical assessment.
  • We do not promise magic solutions.

Healthcare perspective

When coordination with medicine or urology may be useful

Delayed ejaculation can be related to medication, alcohol use, diabetes, neurological problems, prostate or bladder surgery, changes in sensitivity, age or other physical factors.

For that reason, if signs appear during assessment that require medical review, we will say so clearly. At Ocnos we work from health psychology, but we understand sexual health as an area where it can sometimes be useful to bring together different professional perspectives.

Do not change or stop medication on your own. If you suspect that a medicine may be affecting your sexual response, the right step is to discuss it with the professional who prescribed it.

Hector is amazing. As a parent, supporting a 10-year-old child through life’s challenges sometimes feel overwhelming, and having the right guidance makes a big difference. He is helping me better understand my daughter needs, and how I can best support her as a parent. What I appreciate most is that he doesn’t just offer advice; he helps me see situations from a different perspective. His guidance is really valuable to me and has made a big difference in the way I approach parenting. I highly recommend him. Thank you Hector.

Booking my first appointment with Hector was one of the best choices I made. It took me many years to take the first step, and I am very glad I did, and that it was Hector the person helping me on my journey to overcome the obstacles preventing me from moving forward in my life. With just a few sessions and proper guidance and self work I made a lot of progress that I am satisfied with. I wanted to help myself, but I needed a guide to help me help myself. Hector was that person. Very grateful and thankful, and highly recommended. Thank you 🙏

Ocnos Psychology in Palmones is a truly welcoming, professional clinic, with a calm environment and ample parking that makes visiting feel easy from the outset. I have personally found Hector’s support invaluable in deepening my understanding of neurodiversity and developing strategies that have helped me both personally and professionally. He is kind, compassionate, highly knowledgeable and wonderfully adaptive to each person’s needs. He creates a safe space where you feel genuinely heard, supported and empowered to build on your strengths. He offers sessions in both English and Spanish, and his background as a teacher makes him exceptionally well placed to work with both children and adults. The highest compliment I can give is that, as a coach myself, I would actively refer clients to Hector if they wished to explore a neurodiversity assessment or diagnosis. Highly recommended.

I had the best experience at Ocnos Clinic. I had my sessions in English with Héctor, and from the very first session I felt comfortable, listened to, and genuinely understood. What stood out to me most was how knowledgeable and dedicated Héctor is. It’s clear that he genuinely cares about his clients and goes above and beyond to help. His approach has really helped me better understand and manage my anxiety, and I’ve already noticed a positive difference after just a few sessions. I would highly recommend Héctor and Ocnos Clinic to anyone looking for professional, compassionate support. It’s absolutely worth it.

Frequently asked questions

Frequently asked questions about delayed ejaculation

Understanding the problem

What is delayed ejaculation?

Delayed ejaculation is difficulty ejaculating, or needing a long time, effort or stimulation to do so. It is not defined only by time, but by the distress it causes, the frustration, avoidance or impact on the relationship.

Is it the same as anorgasmia?

Not exactly. Anorgasmia refers to difficulty reaching orgasm. Delayed ejaculation focuses on difficulty ejaculating. In some people the two experiences overlap, which is why it is important to assess the case carefully.

Why can I ejaculate alone but not with a partner?

This can happen because of pressure, self-monitoring, fear of taking too long, difficulty letting go, type of stimulation, very specific sexual habits or tension in the relationship. It does not necessarily mean lack of desire or lack of attraction.

Causes and assessment

Is delayed ejaculation always psychological?

No. It can have psychological, relational, medical, pharmacological or mixed factors. That is why we assess anxiety, pressure, habits, relationship context, medication, general health and any signs that may require medical review.

Can medication have an effect?

Yes. Some medicines, especially certain antidepressants, can affect ejaculation or orgasm. Medication should not be changed or stopped without medical advice; the right step is to speak with the professional who prescribed it.

Do I need to see a doctor before starting therapy?

Not always, but it may be advisable if the difficulty appeared suddenly, coincides with new medication, or there is pain, urinary symptoms, changes in sensitivity, previous medical conditions, substance use or medical concerns. Psychological therapy can complement, but not replace, a necessary medical assessment.

Psychological treatment

Does therapy involve sexual exercises in the consultation room?

No. No sexual practices are carried out in consultation. The work is based on clinical conversation, psychological assessment, psychoeducation, reducing pressure, home-based strategies when appropriate and reviewing progress respectfully.

Can I come with my partner?

Yes. This can be useful when the difficulty is creating pressure, distance, frustration or misunderstandings. It is also possible to start individually if the person prefers to understand what is happening first.

How much does a session cost?

The individual session lasts approximately 60 minutes and costs €70. If the work is carried out as couples therapy, the session lasts approximately 90 minutes and costs €110.

How many sessions are usually needed?

There is no fixed number. It depends on whether the difficulty is recent or long-standing, whether there are medical or pharmacological factors, whether it appears only with a partner, whether there is intense anxiety or whether there are other associated sexual difficulties. After the first assessment we can guide you more clearly.
